How to Apply for a Rebate

Learn who is eligible for a rebate, any stipulations, and instructions for submitting your application!

Any resident of Lake County, FL! Rebates provided by The Stacks Foundation, Inc. are intended to help those on a limited income with the greatest need. If your annual household income is over $75,000, we encourage you to refrain from applying for a rebate to save the funds for those less fortunate families. Thank you.
You must be a Lake County resident. Proof of residence will be required on the rebate application.
Rebates are ONLY valid to help offset the cost of spay and neuter procedures. They cannot be used towards the cost of vaccinations.
- Male Cat: $50
- Female Cat: $60
- Male Dog, Under 40 pounds: $75
- Male Dog, Over 40 pounds: $94
- Female Dog, Under 40 pounds: $85
- Female Dog, Over 40 pounds: $100

Please allow 6-8 weeks for processing of your rebate check.
You may take your pet to any veterinarian of your choosing for their procedure. A list of veterinarians that have previously supported The Stacks Foundation can be found here.

Ready to Apply? Listed below are the items needed on the application

- Your full name
- Your physical address
- Your mailing address
- Cat or Dog
- Male or Female
- If dog, over or under 40 pounds
Document must contain the following printed information:
- Your full name
- Your physical address (that matches application)
- Issue date within 60 days of your application
- Examples include: utility bill, cable bill, pay stub, etc.
Rebate applications must be received within 60 days of each pet's surgery Invoice must show:
- Your name
- Your address
- Your pet's name
- Date
There will be a small $3 processing fee to be paid at the time of application submission.
